    The Lambeth Advance Care Planning project ran from 2019 until 2022 using Big Lottery funding. We were a partnership of local organisations who worked with the national charity Compassion in Dying to support people to think about their future care and treatment and record them so the people around them and the health and care system know their wishes in the event that they are no longer able to communicate.

    Our work focuses on community and secondary care services, prevention and early intervention and addressing mental health inequalities in Lambeth.

    We are represented on 30 different committees, boards and working groups. We use this role to share patient/service user feedback, inform discussion, influence processes and, in some cases, participate in decision making.
