Lambeth Advance Care Planning Consortium is looking for volunteers to help local residents to record their future treatment and care preferences, either for the end of life or if they become unable to make or communicate decisions.
Faced with the realities of the coronavirus pandemic, the importance of considering what matters to us in relation to our quality of life, medical treatment and health and care priorities has never been more essential.
Advance Care Planning Supporters will provide one-to-one help for people - including those who may be particularly vulnerable to Covid-19 - to explore these questions, think ahead and create an advance care plan to record their future treatment and care wishes.
Advance care plans are one of the items that those being shielded from coronavirus are recommended to include in a packed bag ready for hospital admission. You will be enabling people to create these important documents, helping them to ensure that their voice is heard when it matters most.
The role is flexible but we ask Supporters to commit to participating in the project for at least six months and to help support at least two people in up to six one-to-one telephone sessions each. Please note that references and an enhanced DBS are required for this role.
An initial one-day training course will be delivered online regularly from 21 April 2020..
