Age UK Lambeth Launches New Welcome Home from Hospital Service

Services include minor adjustments to home and link ups with other community services

Age UK Lambeth has launched it's new Welcome Home from Hospital service, supporting people with discharge from hospital care back to their residence. 

Alongside Adult Social Service discharge teams at Guy's and St. Thomas's, King's College and St. George's hospitals, Hospital 2 Home workers will support patients when they are due to be discharged from hospital. They will support people either in getting home or once they are there and can also help with ensure minor adjustments (like moving furniture) have been done by the day you arrive home, ring your care agency so they know you have left hospital, and more.

Annual General Meeting 2024

We held our Annual General Meeting (AGM) for 2024 on Wednesday, 4 December.
