Lambeth is changing the way care is delivered by offering more services in the community- closer to home and outside of hospital settings.
The aim is to develop neighbourhood teams that bring together professionals to help people stay healthy, live more independently and avoid unnecessary hospital visits by getting the right support in the local community.
At first these neighbourhood teams will focus on:
- Children and young people with complex needs – for example those with long-term health-conditions, mental or emotional needs, neurodiversity, behavioural needs and social or economic pressures within their family.
- People who have multiple long-term-conditions -such as diabetes, high blood pressure or heart disease.
- People who are frail -This may include slow walking pace, difficulty recovering from illness, having low energy or weakness, or a higher risk of falls. and other challenges which may lead to hospital admission. Frailty can occur at any age.
- People needing mental health support -for example with depression, anxiety, and trauma.
