    End of life care is support for people who are in the last months or years of their life. Resources on advance care planning, and the guidance on the NHS website helps you know what to expect, how to plan ahead and how to manage your decisions.

    We carried out an Enter and View to take a closer look at community-based mental health services. The project was based on a recognition of the large numbers of people who rely on primary care and community services as an important means of support.

    We carried out an evalution of the first two years of Project Smith, where local people worked together with NHS Lambeth CCG and Lambeth Council, recognising that the community and people are the assets and key to good health and wellbeing.

    A pilot project aimed to test out an approach for building a picture of older people’s experiences of health and social care services following a stay in hospital.

    We evaluated the adaptation of an existing hospital-based initiative designed by the Guy's and St Thomas' Trust, to help carers of elderly family members with dementia prepare for their return home after a stay in hospital.

    We have published our latest round up of feedback from local people features experiences of a range of services. We received 63 calls between October 2018 to March 2019 (Quarter three and four of 2018/19).

    Lambeth Council has reviewed all the fees and charges for adult social care in Lambeth, and are now seeking views from Lambeth residents as part of their decision-making process.

    Over the past year, we have been working with Lambeth Council on a project looking into how home care can be improved. The aim was to use design approaches to find ways for home care to be more joined up with other services and to empower people.

    Over the summer, we hosted two student volunteers, Fahmida and Hager, from the Nuffield Research Programme. They supported our Engagement Manager Natalia’s two projects for children and young people in Lambeth. Hear from Fahmida about her experience.

    In partnership with the Lambeth Council and Lambeth Made we evaluated the Early Help pilot in Streatham. Reframing Early Help Programme aims to improve the Early Help offer to enable families to receive high quality, whole-family support.

    We planned a series of Enter and View visits to community-based mental health services including supported accommodation, employment and training and clinical treatment services, including Penrose Options.

    We ran a series of Enter and Views into community services. Connelly House, run by Cyrenians, offers 24-hour supported accommodation for those with complex mental health needs.

    in October 2016 we invited all extra care residents in Lambeth to a tea and chat event at one of the extra care facilities. We wanted to find out how residents were feeling one year on from our review and what difference the grants may have begun.

    We undertook a review of dementia services in the Clapham Park area of Lambeth during the second half of 2014, in order to gain an insight into the experiences of local residents with dementia and their carers (both paid and unpaid)

    We did an Enter and View visits at Lingham Court, an extra care housing facility for 30 tenants, providing care and support to older people including wheelchair users and five residents living with dementia.

    In this presentation to the Lambeth Council Health & Wellbeing Board, we highlighted why social relations are important, in relation to our priority areas including Poor Mental Health, Community based care and Cuts to welfare and care budgets.

    In 2015, we worked with Brook – the national sexual health charity to pilot ways of engaging with young people. A core volunteer group was recruited and trained as researchers to seek out health and social care experiences of their peers.

    The Community Connectors is one of two programmes in Project Smith, an NHS funded community development programme. It is for Lambeth residents who are active in their community or would like to be active in their community.

    Here you can find all past issues of the LLDA newsletter. The newsletter has useful information for Lambeth people with Learning disabilities. It also introduces new services and talks about issues that local people with learning disabilities share.

    Central London Works (CLW) is a voluntary employment support programme designed to help residents of London boroughs including Lambeth, with a health condition (or who are long-term unemployed) manage their health conditions and look for employment.

    Advocacy helps you to have your say. From 1st July 2020 all statutory Independent Advocacy for people aged 16 and over in Lambeth is provided by the Advocacy Hub on behalf of Connect Lambeth. This service is funded by Lambeth Council.

    We spoke to social care staff about the health assessment process of children who are in the care of Lambeth Council. The aim was to identify how the service could be improved and reduce delays in assessments.
